We are currently seeking a dedicated and experienced Qualified Social Worker to join our Community Mental Health Team (CMHT) in Salford. As a key member of the team, you will be responsible for providing high-quality social work services to individuals experiencing mental health challenges within the community.

Key responsibilities and duties include:

Conducting comprehensive assessments of service users' needs and developing personalized care plans. Providing therapeutic interventions and support to promote recovery and well-being. Collaborating with multi-disciplinary teams, including healthcare professionals and community partners, to ensure holistic care. Advocating for service users' rights and empowering them to make informed decisions about their care. Participating in safeguarding procedures and risk assessments to ensure the safety of vulnerable individuals.

The ideal candidate for this position will possess:

A recognized Social Work qualification and registration with Social Work England. Demonstrated experience working in a similar role within a mental health setting. Strong assessment and analytical skills, with the ability to develop effective care plans. Excellent communication and interpersonal skills, with a compassionate and empathetic approach. Ability to work autonomously and collaboratively within a team environment.

Additional requirements:

Valid driver's license and access to a vehicle for travel within the community. Flexibility to work occasional evenings or weekends to meet the needs of service users. We are currently seeking two experienced and motivated Qualified Social Workers to join Salford's Community Mental Health Services as Mental Health Commissioning and Review Officers.

These vital roles will be based within integrated Community Mental Health Teams (CMHTs) at Prescott House and Ramsgate House, and will play a key part in the statutory review of mental health care packages, including 12-month reviews in line with the Care Act 2014 and Section 117 of the Mental Health Act.

Key Responsibilities:
Carry out statutory Care Act and s.117 reviews of mental health support packages.

Promote person-centred, strength-based practice.

Work collaboratively with colleagues across multiple CMHTs to support a consistent and high-quality review service.

Contribute to effective discharge planning and resource management.

Liaise with service users, families, care providers, and other agencies.

Maintain accurate and timely case records using electronic case management systems.

Requirements:
Qualified Social Worker (Degree/DipSW/CQSW)

Current Social Work England registration

Substantial post-qualifying experience in mental health adult social care

Knowledge and application of the Care Act, Mental Health Act, and Safeguarding procedures

Ability to work independently, manage competing demands, and meet deadlines

Excellent interpersonal, communication, and organisational skills

A valid UK driving licence and access to a vehicle (travel required across Salford and out-of-area placements)

Additional Information:
These roles fall under Section 75 arrangements, with practitioners embedded in Greater Manchester Mental Health NHS Foundation Trust.

3 years of reference checks
