We are currently seeking a dedicated and experienced Qualified Social Worker to join our Community Mental Health Team (CMHT) in Salford. As a key member of the team, you will be responsible for providing high-quality social work services to individuals experiencing mental health challenges within the community.
Key responsibilities and duties include:
- Conducting comprehensive assessments of service users' needs and developing personalized care plans.
- Providing therapeutic interventions and support to promote recovery and well-being.
- Collaborating with multi-disciplinary teams, including healthcare professionals and community partners, to ensure holistic care.
- Advocating for service users' rights and empowering them to make informed decisions about their care.
- Participating in safeguarding procedures and risk assessments to ensure the safety of vulnerable individuals.
- A recognized Social Work qualification and registration with Social Work England.
- Demonstrated experience working in a similar role within a mental health setting.
- Strong assessment and analytical skills, with the ability to develop effective care plans.
- Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
- Ability to work autonomously and collaboratively within a team environment.
- Valid driver's license and access to a vehicle for travel within the community.
- Flexibility to work occasional evenings or weekends to meet the needs of service users.
