We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Community Prevention Team Hybrid in Nottingham. In this role, you will play a crucial part in supporting individuals and families within the community to prevent crises and promote well-being.
Key responsibilities and duties:
Conducting assessments and developing care plans for vulnerable individuals. Providing support and guidance to service users to enhance their quality of life. Collaborating with multi-disciplinary teams and external agencies to coordinate holistic care. Participating in safeguarding procedures and ensuring the welfare of at-risk individuals. Contributing to the development and implementation of preventative strategies within the community.Essential qualifications and skills:
Qualified Social Worker with relevant registration and experience in a similar role. Strong communication and interpersonal skills to engage effectively with diverse service users. Ability to work autonomously and make sound decisions under pressure. Knowledge of relevant legislation and best practices in social work.
