We are currently seeking a dedicated and passionate Children's Social Worker to join our CWD Team in Wiltshire. As a key member of the team, you will be responsible for providing high-quality support and intervention to children with disabilities and their families.
Key responsibilities and duties include:
Assessing the needs of children with disabilities and developing care plans Working collaboratively with families, schools, and other professionals to ensure the best outcomes for children Conducting home visits and meetings to monitor progress and provide support Advocating for the rights and well-being of children with disabilities Participating in case conferences and reviews to evaluate and adjust care plansThe ideal candidate will possess the following essential qualifications and skills:
Qualified Social Worker with HCPC registration Experience working with children with disabilities in a social work setting Strong assessment and case management skills Excellent communication and interpersonal skills Ability to work effectively in a multidisciplinary teamAdditional requirements for this role include a valid driver's license and access to a vehicle for travel within the community.
