Eden Brown Synergy is currently recruiting for an experienced Qualified Social Worker to join the Hospital Social Work Team based at St Richard's Hospital, West Sussex.

The Hospital Social Work Team is committed to making a positive difference to the people of West Sussex. You will work with individuals within the hospital setting and those recently discharged, ensuring they receive the care, support, and services needed to maximise their independence and wellbeing.

This is a varied and fast-paced role, offering the opportunity to work closely with healthcare professionals, community resources, and voluntary sector organisations. The team is highly supportive and committed to the ongoing development of its practitioners.

Key ResponsibilitiesDeliver a statutory social work service within a hospital settingUndertake strengths-based assessments and care planningSupport individuals to regain independence following hospital admissionWork collaboratively with colleagues, carers, health professionals, and external agenciesParticipate in safeguarding enquiries and risk assessmentsAttend ward rounds, family meetings, and multidisciplinary team discussionsMaintain accurate and compliant case recordsEssential Skills & ExperienceQualified Social Worker with Social Work England registrationExperience undertaking strengths-based assessments and care planningKnowledge of relevant legislation and safeguarding proceduresExperience applying risk enablement and evidence-based practiceStrong analytical and decision-making skillsAbility to mentor and support less experienced practitionersExcellent communication and stakeholder management skillsCompetent IT and case recording skillsBenefits of Working with Eden Brown Synergy
