Qualified Social Worker - Hospital Team (Hybrid)
West Sussex | St Richard's Hospital | Up to £38 per Hour
Eden Brown Synergy is currently recruiting for an experienced Qualified Social Worker to join the Hospital Social Work Team based at St Richard's Hospital, West Sussex.
Pay Rates
- £38 per hour
OR - £35 per hour plus £150 per week expenses
Working Arrangements
- Hybrid working model
- 2-3 days per week onsite at St Richard's Hospital
- Remaining time can be worked remotely
- Attendance required for ward visits, assessments, family meetings, duty work, and multi-disciplinary team meetings
About the Role
The Hospital Social Work Team is committed to making a positive difference to the people of West Sussex. You will work with individuals within the hospital setting and those recently discharged, ensuring they receive the care, support, and services needed to maximise their independence and wellbeing.
This is a varied and fast-paced role, offering the opportunity to work closely with healthcare professionals, community resources, and voluntary sector organisations. The team is highly supportive and committed to the ongoing development of its practitioners.
Key Responsibilities
- Deliver a statutory social work service within a hospital setting
- Undertake strengths-based assessments and care planning
- Support individuals to regain independence following hospital admission
- Work collaboratively with colleagues, carers, health professionals, and external agencies
- Participate in safeguarding enquiries and risk assessments
- Attend ward rounds, family meetings, and multidisciplinary team discussions
- Maintain accurate and compliant case records
Essential Skills & Experience
- Qualified Social Worker with Social Work England registration
- Experience undertaking strengths-based assessments and care planning
- Knowledge of relevant legislation and safeguarding procedures
- Experience applying risk enablement and evidence-based practice
- Strong analytical and decision-making skills
- Ability to mentor and support less experienced practitioners
- Excellent communication and stakeholder management skills
- Competent IT and case recording skills
