Qualified Social Worker (QSW) - Adults Team | Wokingham Borough Council | £38.97/hr
Location: Wokingham
Rate: £38.97 per hour
Working Arrangement: Hybrid - Office day required on Tuesdays
Contract: Ongoing Locum
Start Date: ASAP

Overview:
Wokingham Borough Council is seeking an experienced and motivated Qualified Social Worker to join their Adults Team. This is a hybrid role, focused on funding assessments, partnership working, and contributing to service-wide improvements.

Key Responsibilities:
Investigate referred cases and assess eligibility for health funding, including Continuing Healthcare (CHC), Section 117, and other frameworks.

Make clear recommendations on funding eligibility and propose funding splits within established policies.

Carry out complex assessments in collaboration with clinicians and care professionals.

Support the Team Manager in operational delivery and team performance oversight.

Assist in the design and delivery of training for council teams and occasionally NHS/health partners on CHC, S117, and other adult and children's funding streams.

Requirements:
Qualified Social Worker with Social Work England registration

Extensive experience in adult social care and health funding assessments

Strong understanding of CHC, S117, and relevant funding legislation

Able to work collaboratively across services and disciplines

Excellent written, analytical, and communication skills

Job Title: Adult Social Worker - Adult Social Care Team

To work within the Adult Social Care team 18+, undertaking direct work with customers who have a learning disability, physical disability, long term conditions, young carers and people transitioning into adulthood and older people. The main aim of the service is to enable people to live safely in the community, maximising their strengths and abilities to live as independently as they can, and supporting carers to continue in their caring role as long as they wish to and are able to.

Requirement:

Qualified social worker with 3 + years' experience of working in adult social care
Able to be office based a minimum 1 x weekly and complete visits on other days
Car driver with access to a car
Experience - Dementia , drug/alcohol related issues, Homelessness ,Hoarders ,S42 ,carers assessment .community work and multi disciplinary work.

Adults with learning disabilities and physical disabilities

Individuals with long-term conditions

Young carers and those transitioning into adulthood

Older people and their carers

You will play a crucial role in enabling people to live safely and independently in the community, while also supporting carers in their vital role.

Key Responsibilities:

Conduct Care Act assessments and reviews

Work in a strengths-based and person-centred way

Develop and review care and support plans

Promote independence and prevent the need for long-term care

Support transitions from childhood to adult services

Liaise with health, community, and voluntary sector partners

Requirements:

SWE registration

Experience working within adult social care

Knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures

Strong assessment and communication skills
