Walsall Council is seeking an experienced Qualified Social Worker to join their Hospital Discharge Team. This is a fantastic opportunity to work as part of a supportive multi-disciplinary team (MDT), focused on delivering timely and person-centred hospital discharges.
Key Responsibilities:
Complete initial and subsequent reviews of care packages arranged after hospital discharge (home or short-term placements)
Assess needs and revise support plans where necessary
Identify and refer for Occupational Therapy (OT) input as needed
Promote independence through assistive technology and community asset signposting
Carry out Mental Capacity Assessments (MCAs) and risk assessments
Make referrals to extra care housing and in-house provider services when appropriate
Ensure all work is compliant with statutory duties and Care Act principles
Requirements:
Qualified Social Worker with active Social Work England (SWE) registration
Experience in hospital discharge or similar adult social care setting
Strong working knowledge of the Care Act, MCA, and safeguarding processes
Ability to work collaboratively within an MDT and manage caseloads effectively
