We are currently seeking a dedicated and experienced Adult's Social Worker to join our Mental Health Team in Walsall. As a key member of the team, you will be responsible for providing high-quality support and interventions to adults with mental health issues, ensuring their well-being and promoting independence.
Key responsibilities and duties include:
- Conducting assessments and developing care plans for individuals with complex needs
- Working collaboratively with multi-disciplinary teams to coordinate and deliver effective support services
- Providing crisis intervention and safeguarding vulnerable adults
- Advocating for clients and ensuring their rights are upheld
- Participating in case conferences and reviews to monitor progress and adjust interventions as needed
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with current registration with the HCPC
- Demonstrated experience working with adults with mental health issues in a social work setting
- Strong assessment and analytical skills, with the ability to develop and implement effective care plans
- Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders
- Ability to work autonomously and as part of a team, with a proactive and solution-focused approach
Additional requirements for this role include a valid driver's license and access to a vehicle, as home visits may be required.
