We are seeking a dedicated and experienced SWE registered Social Worker to join our Mental Health team, supporting adults with mental health needs to achieve positive outcomes and maintain independence within their communities.
Working as part of a multidisciplinary team, you will contribute to the delivery of high-quality, person-centred services, undertaking assessments, care planning, safeguarding responsibilities, and partnership working with health and community organisations.
Key Responsibilities
Conduct strengths-based assessments and reviews.
Develop and implement person-centred care and support plans.
Work collaboratively with health professionals, service users, carers, and partner agencies.
Manage a caseload of adults with complex mental health needs.
Ensure compliance with relevant legislation, policies, and safeguarding procedures.
Promote recovery, independence, and community inclusion.
Requirements
Relevant professional qualification (e.g., Social Work, Occupational Therapy, or equivalent).
Experience working within Adult Social Care and/or Mental Health Services.
Knowledge of relevant legislation including the Care Act, Mental Health Act, and safeguarding frameworks.
Strong assessment, communication, and case management skills.
Ability to work effectively within a multidisciplinary environment.
Location: Lambeth Living Well Centre, SW2 1RW
Rate: £32.66 per hour umbrella
Office Attendance: 3 days per week
