Liverpool - Social Worker - Mental Health Hospital Discharge team

We are seeking a dedicated and compassionate Qualified Social Worker to join our Mental Health Hospital Discharge Team in Liverpool. This role is pivotal in supporting individuals with mental health challenges as they transition from hospital to community-based care. You will work collaboratively with multidisciplinary teams to ensure a seamless discharge process, promoting recovery and independence for our clients.

Your key responsibilities will include:

  • Conducting assessments and developing discharge plans in partnership with patients, families, and healthcare professionals.
  • Providing emotional support and practical assistance to patients and their families throughout the discharge process.
  • Coordinating with community services to ensure appropriate follow-up care and support.
  • Monitoring and evaluating the effectiveness of discharge plans, making adjustments as necessary.
  • Maintaining accurate and up-to-date records in compliance with relevant legislation and policies.

The ideal candidate will possess the following qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work or equivalent).
  • Current registration with Social Work England.
  • Experience working in mental health settings, particularly with hospital discharge processes.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
  • Excellent organizational skills and the ability to manage multiple cases effectively.
