We are currently recruiting for a dedicated and highly organised EHC Plan Co-ordinator to join the Special Educational Needs (SEN) Service at Tower Hamlets Council. This key role is responsible for administering and co-ordinating Education, Health and Care (EHC) needs assessments and plans for children and young people aged 0-25 in line with the Children and Families Act 2014 and SEND Code of Practice.

You'll be the first point of contact for families and professionals, ensuring children and young people are placed at the heart of a person-centred approach, and that statutory duties are met with efficiency and empathy.

Key Responsibilities
* Manage a specific caseload of EHC assessments and plans.
* Act as the key liaison for parents, schools, and professionals throughout the EHC process.
* Coordinate and chair person-centred EHC Plan meetings.
* Maintain compliance with statutory timeframes and ensure all documentation is accurate and up to date.
* Coordinate admissions and transfers of children with EHC Plans, including out-of-borough placements.
* Provide advice and training to schools and parents on SEN processes and procedures.
* Input and manage data using Synergy and other relevant systems.
* Contribute to continuous service improvement across the SEN Section.

About You
To succeed in this role, you'll need:
* Proven experience in administering complex casework within education, SEN or local authority settings
* Strong working knowledge of SEND legislation and statutory guidance
* Excellent communication and interpersonal skills, with the ability to build trust with families and professionals
* Ability to work under pressure and manage a demanding caseload within statutory deadlines
* Strong IT and administrative skills, with the ability to accurately manage sensitive data
* A person-centred, empathetic approach to service delivery
* Enhanced DBS on the children's barred list (or willingness to obtain)

* Previous experience working in a local authority SEN or EHCP coordination role
* Knowledge of local specialist education provision
* Experience of chairing or leading multi-agency meetings

Key Responsibilities:
* Act as the key point of contact for young people aged 14-25 with complex learning difficulties and/or disabilities, supporting their transition into further education, training, or employment.
* Carry out person-centred Education, Health & Care (EHC) assessments and ensure recommendations reflect educational, health, and care support needs.
* Lead and coordinate transition planning (starting from Year 9), ensuring long-term, outcome-focused placement strategies.
* Provide information, advice, and guidance (IAG) to young people and their families regarding transition pathways.
* Monitor placements and challenge provisions if needs are not being met appropriately.
* Maintain accurate placement/progress records, contribute to placement panels, and manage personal budgets where applicable.
* Collaborate across borough services (health, education, social care) and external partners to deliver integrated support packages.

Requirements:
* Significant experience in Preparing for Adulthood (PfA) roles or transition teams.
* Strong understanding of EHCP processes, person-centred planning, and post-16 SEN provision.
* Sound knowledge of legislation relevant to SEN, including the Children and Families Act, Care Act, and SEND reforms.
* Experience working with young people with complex needs and coordinating multi-agency support.
* Excellent organisational, communication, and IT skills.
* Enhanced DBS required (Adults & Children).

* Familiarity with the Mental Capacity Act in relation to post-16 young people.
