We are seeking a dedicated Adult's Social Worker to join our Locality Team in North Lincolnshire. In this role, you will be responsible for providing high-quality social work services to adults in the community, ensuring their safety, well-being, and independence.

Key responsibilities and duties include:

Conducting assessments and developing care plans for adults with complex needs. Working collaboratively with multi-disciplinary teams to coordinate and deliver support services. Undertaking safeguarding investigations and interventions as required. Advocating for clients to access appropriate resources and services. Participating in case conferences and reviews to monitor progress and adjust care plans.

The ideal candidate will possess:

A degree in Social Work and registration with Social Work England. Demonstrated experience in adult social work, preferably within a locality team setting. Strong assessment, communication, and decision-making skills. Ability to work autonomously and as part of a team in a fast-paced environment. Commitment to upholding ethical standards and promoting social justice.

Additional requirements:
