We are seeking a dedicated Adult's Social Worker to join our Locality Team in North Lincolnshire. In this role, you will be responsible for providing high-quality social work services to adults in the community, ensuring their safety, well-being, and independence.

Key responsibilities and duties include:

  • Conducting assessments and developing care plans for adults with complex needs.
  • Working collaboratively with multi-disciplinary teams to coordinate and deliver support services.
  • Undertaking safeguarding investigations and interventions as required.
  • Advocating for clients to access appropriate resources and services.
  • Participating in case conferences and reviews to monitor progress and adjust care plans.

The ideal candidate will possess:

  • A degree in Social Work and registration with Social Work England.
  • Demonstrated experience in adult social work, preferably within a locality team setting.
  • Strong assessment, communication, and decision-making skills.
  • Ability to work autonomously and as part of a team in a fast-paced environment.
  • Commitment to upholding ethical standards and promoting social justice.
