An excellent opportunity has become available for an Occupational Therapist to join the Front Door Team at the Royal Borough of Windsor & Maidenhead.

This is a rewarding role for an experienced Occupational Therapist who is passionate about supporting adults to maintain their independence through timely, person-centred assessments and interventions.

The Role

As part of the Front Door Team, you will provide an outcome-focused Occupational Therapy assessment service to adults with physical disabilities and age-related conditions. You will work closely with individuals, their families, and multidisciplinary professionals to promote independence and deliver effective, strengths-based solutions.

Your responsibilities will include:

Carrying out comprehensive Occupational Therapy assessments.Supporting adults with physical disabilities and age-related needs.Recommending equipment, adaptations, and moving and handling solutions where appropriate.Developing person-centred care and support plans.Working collaboratively with health, social care, and community partners.Managing a varied caseload while ensuring high-quality and timely interventions.Essential Requirements

You will be based at the Hackney Service Centre, with regular home visits across the borough.

Key ResponsibilitiesDevelop and implement person-centred Reablement Therapy Plans.Identify meaningful goals and outcomes that maximise independence.Monitor progress and review therapy interventions throughout the reablement period.Work collaboratively with Reablement Support Workers, Occupational Therapy Assistants, Senior Practitioners and Adult Social Care teams.Provide professional advice and guidance to reablement providers.Promote a "doing with" rather than "doing for" approach, encouraging service users to regain everyday living skills.Support safe hospital discharge and prevent unnecessary long-term care packages.Complete accurate documentation and contribute to outcome-focused reviews.About the Service

The Reablement at Home service offers up to six weeks of intensive, therapy-led support to adults eligible for reablement services.

Service users may receive up to four visits per day, seven days a week, with support reducing as independence improves.

Social Worker required for work within the Permanence Team in the Dorset area. Long term contracts available n the beautiful county of Dorset. 3 months initial contract, 37 hours a week. Competitive rates of pay up to 42ph. The team has a strong management structure. Flexible, hybrid working arrangements available

An exciting opportunity as arisen for an experienced social worker in the Permanence Team in the Dorchester area of Dorset. Your duties will include, pathway plans, visiting to children in care and care leavers, supporting young people, handle extremely complex cases with the assistance of management team. Maintaining close positive links with outside agencies and service providers within the community is a priority.
It is essential that candidates have good communication and strong report writing skills.

You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
