? JOB ADVERT - Qualified Social Worker (Hospital Discharge) - Slough - £36.81 p/h

We are currently looking for an experienced Qualified Social Worker to join the Hospital Discharge Team in Slough. This is a fast-paced and rewarding role based primarily at Wexham Park Hospital.

Key Responsibilities:
Manage hospital discharges within strict 3-6 day timelines

Undertake Care Act assessments, safeguarding enquiries, and discharge planning

Work collaboratively with health colleagues to ensure safe and timely discharges

Attend on-site assessments and meetings as required

Complete duty once per week on a rota basis

Handle safeguarding concerns related to neglect or abuse occurring within the hospital

Requirements:
A minimum of 4 years post-qualified experience

Strong knowledge of hospital discharge processes and community services

Ability to work independently under pressure

Experience using LAS (Liquidlogic Adults' System) is highly desirable

Must be able to travel to Wexham Park Hospital - a parking permit can be arranged
