Key responsibilities and duties:

Provide comprehensive social work support to young people transitioning into adulthood, including assessment, planning, and intervention. Work collaboratively with multi-disciplinary teams and external agencies to ensure the holistic needs of young adults are met. Advocate for the rights and best interests of young people, empowering them to make informed decisions about their futures. Conduct risk assessments and develop safeguarding plans to protect vulnerable individuals. Keep accurate and up-to-date records, complying with legal and regulatory requirements.

Essential qualifications and skills:

Qualified Social Worker with a valid registration with Social Work England. Demonstrated experience working with young people aged 16-25 in a social work setting. Strong assessment and analytical skills, with the ability to develop and implement care plans tailored to individual needs. Excellent communication and interpersonal skills, with the ability to build rapport with young people and their families. Excellent communication, organisational, and multi-agency working skills

Enhanced DBS on the Update Service preferred
