Role Overview:
Birmingham City Council is seeking an experienced and dedicated Qualified Social Worker (QSW) to join our Discharge to Assess South Community Team, specifically working within the Summer Hill Terrace area. This role focuses on providing social work support as part of the homeless/housing pathway for individuals discharged from hospital.
You will be responsible for conducting assessments and support planning within the Conversations Model framework, handling cases that are often complex due to the mix of housing, safeguarding, and substance misuse issues. This is a dynamic, multi-disciplinary environment where you will work closely with healthcare professionals, housing teams, and other specialists to provide tailored support for vulnerable individuals.
Key Responsibilities:
Undertake assessments and support planning for individuals within the Discharge to Assess pathway.
Work with individuals with complex housing needs, substance misuse issues, and safeguarding concerns.
Provide high-quality, person-centered social work support using the Conversations Model in line with Birmingham City Councils practices.
Collaborate with healthcare professionals, housing services, and other relevant agencies to ensure effective care planning.
Ensure compliance with safeguarding protocols, identifying and addressing risks as needed.
Provide clear and comprehensive reports, maintaining accurate case records in line with council policies.
The Ideal Candidate:
Qualified Social Worker with post-qualification experience in adult social work, particularly in discharge or homelessness pathways.

Ability to manage complex cases in a multi-disciplinary setting.
Sound knowledge of Care Act assessments, Mental Capacity Act, and relevant legislation.
