We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team in Slough in a Hospital Discharge position. As a key member of our social work team, you will play a crucial role in facilitating safe and timely discharges for patients, ensuring they receive appropriate support and services upon leaving the hospital.
Key responsibilities and duties:
Conduct assessments and create care plans for patients being discharged from the hospital Coordinate with healthcare professionals, community services, and families to ensure a smooth transition for patients Advocate for patients' needs and rights, ensuring they receive the necessary support and resources Provide emotional support and guidance to patients and their families during the discharge process Maintain accurate and up-to-date records of all interactions and interventionsEssential qualifications and skills:

We are currently looking for an experienced Qualified Social Worker to join the Hospital Discharge Team in Slough. This is a fast-paced and rewarding role based primarily at Wexham Park Hospital.
Key Responsibilities:
Manage hospital discharges within strict 3-6 day timelines
Undertake Care Act assessments, safeguarding enquiries, and discharge planning
Work collaboratively with health colleagues to ensure safe and timely discharges
Attend on-site assessments and meetings as required
Complete duty once per week on a rota basis
Handle safeguarding concerns related to neglect or abuse occurring within the hospital
Requirements:
A minimum of 4 years post-qualified experience
Strong knowledge of hospital discharge processes and community services
Ability to work independently under pressure
Experience using LAS (Liquidlogic Adults' System) is highly desirable
Must be able to travel to Wexham Park Hospital - a parking permit can be arranged
