We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team in Slough in a Hospital Discharge position. As a key member of our social work team, you will play a crucial role in facilitating safe and timely discharges for patients, ensuring they receive appropriate support and services upon leaving the hospital.

Key responsibilities and duties:

  • Conduct assessments and create care plans for patients being discharged from the hospital
  • Coordinate with healthcare professionals, community services, and families to ensure a smooth transition for patients
  • Advocate for patients' needs and rights, ensuring they receive the necessary support and resources
  • Provide emotional support and guidance to patients and their families during the discharge process
  • Maintain accurate and up-to-date records of all interactions and interventions

Essential qualifications and skills:

  • Qualified Social Worker with a valid HCPC registration
  • Experience working in a hospital or healthcare setting, preferably in a discharge planning role
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced and high-pressure environment
