Are you passionate about making a meaningful difference in the lives of young people? The Swindon Borough Council is seeking a dedicated and experienced Qualified Social Worker to join our Children's Leaving Care Team. This role is pivotal in supporting young individuals as they transition into adulthood, ensuring they have the guidance and resources they need to thrive in their new independence.

As a Qualified Social Worker in the Leaving Care Team, your key responsibilities will include:

Providing high-quality assessments and support plans for young people aged 16-25 who are leaving care. Building trusting relationships with young people to empower them in their journey towards independence. Collaborating with external agencies and stakeholders to facilitate access to education, employment, and housing opportunities. Monitoring and reviewing care plans regularly to ensure the needs of each individual are met. Advocating for the rights and interests of young people, ensuring their voices are heard in decision-making processes.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (Degree in Social Work or equivalent). Current registration with Social Work England. Experience working with children and young people, particularly those in care or leaving care. Strong communication and interpersonal skills, with the ability to engage effectively with young people and their families. Sound knowledge of relevant legislation, policies, and best practices in children's services.

The role is within in our Leaving Care Service where you will manage a caseload of young people who are preparing to leave care. You will also be working with the allocated Pathway Advisors for the young person who are also based within our service. Our service links in early with the young people transferring for other services and you will lead on the completion of pathway plan assessments and the initial pathway plan. It will remain your ongoing responsibility to maintain the plans in line with the required legislation up until the young person is 18 years old when they will then fully transfer to their allocated Pathway Advisor. In an addition to managing your own caseload you will also be required to be part of our duty system that provides additional support to our young people alongside our Pathway Advisors.


You will be SWE registered, ideally a driver with access to own car and able to reside and work in the UK

Working with the other assistant team managers in ACP, your team will be on Duty one in every four weeks receiving new referrals from MASH to complete statutory assessment and any safeguarding processes to ensure that a child is not at risk of harm. This fast-paced team requires a manager who is able to work to strict deadlines and make safe and robust decisions under pressure. The successful candidate will need to be flexible and able to deal with changing needs and requests at short notice. This is an exciting opportunity for someone to really broaden and develop their skill set and become part of a team committed to change and improving outcomes for children and their families in Swindon.

Candidates must be able to attend the office for 1 full week out of 4 for duty week. Then 1-2 days the other weeks.
