Your duties will include, visiting to children in care and care leavers, supporting young people, developing pathway plans, handling extremely complex cases with the assistance of management team. The role is within in our Leaving Care Service where you will manage a caseload of young people who are preparing to leave care. You will also be working with the allocated Pathway Advisors for the young person who are also based within our service. Our service links in early with the young people transferring for other services and you will lead on the completion of pathway plan assessments and the initial pathway plan. It will remain your ongoing responsibility to maintain the plans in line with the required legislation up until the young person is 18 years old when they will then fully transfer to their allocated Pathway Advisor. In an addition to managing your own caseload you will also be required to be part of our duty system that provides additional support to our young people alongside our Pathway Advisors.

Social Workers will be working in this front line services and need previous experience of working within the Assessment setting within Social Care. The service is provided by Experienced Social Workers, who carry out statutory assessments involving home visits, consulting with other professionals / practitioners and agencies, gathering information for the assessment and analysing the potential risk to any child or young person. You will undertake initial assessments and section 47 Child Protection investigations After the completion of the assessment, the case is either closed or held to the point of transfer to the Long Term Locality Teams.
Candidates will be managing a diverse and complex caseload within deadlines. Constant contact within all teams within Children's & Families Social Care and external agencies to address children's and their families' needs with best possible outcomes. Communication with line managers will allow identification of professional development and training that would be beneficial personally and to the team as a whole.
