We are currently seeking a dedicated and experienced Adult's Social Worker to join our Mental Health Team in North Lincolnshire. As a key member of the team, you will be responsible for providing high-quality social work services to adults with mental health needs in the community.

Key responsibilities and duties include:

Conducting assessments and developing care plans for individuals with complex mental health needs. Working collaboratively with multi-disciplinary teams to ensure holistic support for service users. Providing advocacy and support to empower individuals in making informed decisions about their care and treatment. * Convening and chairing Core Group Meetings

* Organising/managing packages of support and packages of care
