The social worker will act as a Mental Health Practitioner alongside colleagues from South London and Maudsley NHS Foundation and the voluntary sector. They will undertake screening of new referrals while on duty, undertaking strengths-based assessments, support planning and stabilisation, as well as leading on adult safeguarding and Care Act assessments within the multi-disciplinary team. * The PCMHT is based in Downham and offers excellent transport links and free local parking.
