Adult's SW - Older People/Assessment Team - Hertfordshire

We are currently seeking a dedicated and experienced Adult's Social Worker to join our Older People/Assessment Team in Hertfordshire. As a key member of the team, you will be responsible for providing high-quality social work services to older adults in the community.

Key responsibilities and duties include:

  • Conducting assessments to determine the needs of older people and developing care plans accordingly
  • Providing support and guidance to older adults and their families to enhance their well-being and independence
  • Collaborating with healthcare professionals, community organizations, and other stakeholders to ensure holistic care for older people
  • Monitoring and reviewing care plans regularly to ensure they meet the changing needs of older adults
  • Participating in multi-disciplinary meetings and case conferences to coordinate care services effectively

Essential qualifications and skills:

  • Qualified Social Worker with current registration with Social Work England
  • Experience working with older people and a strong understanding of issues affecting this population
  • Excellent assessment and communication skills to engage effectively with older adults and their families
  • Ability to work autonomously and as part of a team in a fast-paced environment
  • Knowledge of relevant legislation and policies related to adult social care
