We are looking for an experienced Occupational Therapist to join Islington's Integrated Discharge Service (IDS) - a fast-paced, supportive team that operates seven days a week. You will play a vital role in supporting safe, person-centred hospital discharges and delivering outcome-focused therapy to residents in their homes.

Key Responsibilities:
Support the Discharge to Assess (D2A) pathway by screening referrals and contributing to discharge planning

Carry out post-discharge assessments in the home environment

Deliver therapeutic interventions aimed at maximising independence and preventing readmission

Recommend and implement equipment, minor adaptations, and environmental changes

Design and implement Reablement Plans and Goals based on a strength-based, person-centred approach

Work collaboratively with multi-disciplinary teams across health and social care

Support residents to achieve outcomes and regain independence in daily living tasks

Requirements:
Qualified Occupational Therapist with relevant HCPC registration

Experience in hospital discharge, community therapy, or reablement

Strong understanding of risk assessment, equipment provision, and rehabilitation techniques

Ability to manage a dynamic caseload in a high-pressure environment

Confident working both autonomously and as part of an integrated team

Knowledge of relevant legislation and good practice standards in adult social care
