Location: Plough Lane, Hereford (covering Herefordshire)
Contract: 3 months
Hours: Full time (37 hours per week)
Pay: £38 per hour (Ltd/Umbrella)
Business Mileage: 45p per mile
Role Overview:
Herefordshire Council's Adults & Communities Care Act Assessment Team (CAAsT) is currently seeking a Senior Practitioner to join our dynamic team. This role involves carrying out short-term assessments and support planning with service users who have recently left the hospital via the Discharge to Assess process and may require ongoing services. As a Senior Practitioner, you will play a key role in delivering high-quality, person-centered, and strength-based services to individuals across Herefordshire.
Key Responsibilities:
Carry out Care Act assessments and support planning for individuals who may require ongoing services after hospital discharge.
Work to set timescales in a fast-paced environment while ensuring services are responsive to individual needs.
Provide day-to-day support, guidance, and supervision to staff, including formal and informal supervision.
Oversee safeguarding, allocations, triaging, and completion of assessments.
Use positive risk-taking and conflict resolution techniques to make effective decisions.
Manage a small caseload, ensuring effective assessment and review processes.
Ensure responses are person-centered, strength-based, safe, and effective.
Apply knowledge of social care practice and legislation, including the Mental Capacity Act (2005) and Continuing Health Care.
Support individuals with a range of needs, including mental health, learning disabilities, physical disabilities, and older people.
What We're Looking For:
Qualified social worker, registered with Social Work England.
Strong knowledge of adult social work and case management.
Experience in hospital discharges, mental health, learning disabilities, and safeguarding.
Excellent organisational, decision-making, and interpersonal skills.
A forward-thinking, proactive approach with the ability to work independently.
Ability to manage staff and oversee safeguarding and assessments.
