Overview:
We are currently seeking a dedicated and experienced Band 6 Specialist Physiotherapist to join the Community Therapy Team West. This is a fantastic opportunity to deliver high-quality physiotherapy interventions within a supportive multidisciplinary environment, working across the community setting in West Sussex.
Key Responsibilities:
* Provide specialist physiotherapy assessment, treatment, and management for a wide range of patients in the community.
* Work as part of a multi-disciplinary team to develop personalised care plans and rehabilitation programmes.
* Support hospital discharge and prevent hospital admission through timely, goal-oriented physiotherapy interventions.
* Liaise effectively with GPs, community nursing teams, and social care colleagues to ensure seamless patient care.
* Maintain accurate clinical records and contribute to service quality improvement activities.
* Participate in the supervision and support of junior staff and students.
Requirements:
* HCPC registered Physiotherapist.
* Previous experience working at Band 6 level in a community physiotherapy setting.
* Excellent assessment and clinical reasoning skills.
* Full UK Driving Licence and access to a vehicle (essential).
* Smart Card (essential).
* SystmOne experience (desirable).

Role Overview:
We are seeking a skilled and motivated Band 6 Physiotherapist to join the Discharge to Assess (D2A) service based within the Community Services team at Musgrove Park Hospital. This role supports patients who have been recently discharged from hospital, enabling safe recovery and reablement within their own homes.
The post holder will work collaboratively with D2A-trained provider agencies to assess patients, deliver targeted rehabilitation, and set realistic SMART goals to maximise independence and support long-term care planning.
Key Responsibilities:
Deliver community-based physiotherapy to adults post-discharge
Carry out functional assessments within the patients home environment
Set patient-led SMART goals and monitor progress toward independence
Work closely with reablement care providers to facilitate seamless support
Identify and recommend ongoing support needs or referrals where appropriate
Contribute to timely and effective discharge planning and recovery pathways
Maintain accurate documentation and communicate effectively within the MDT
