We are currently seeking an experienced Qualified Social Worker (QSW) to join the Older People's Team in Doncaster. This is a fantastic opportunity to make a real difference in the lives of older adults, working within a supportive and dedicated team.
Role Responsibilities:
Conduct assessments and reviews in line with the Care Act 2014
Manage a varied caseload of older people with a range of complex needs
Work collaboratively with health professionals, families, and carers
Ensure safeguarding principles are upheld and applied where necessary
Promote independence and person-centred care planning
Requirements:
Social Work qualification (degree or equivalent)
Current registration with Social Work England
Experience working with older people in a statutory setting
Strong knowledge of relevant legislation and frameworks
Ability to work autonomously and within multidisciplinary teams
