Role: Qualified Social Worker - Older People & Physical Disabilities Service
Location: Southwark, London
Rate: Competitive hourly rate
Contract: 3-month locum (with potential for extension or permanent transition)

The Older People and Physical Disabilities Service in Southwark is seeking a dedicated and experienced Social Worker to join their Case Management Team. This is a rewarding locum opportunity for professionals who are passionate about delivering high-quality adult social care in a supportive and well-structured environment.

Key Responsibilities:

Manage longer-term (6+ weeks), complex cases involving older adults and individuals with physical disabilities.

Work with clients who may have co-occurring mental health or substance misuse needs.

Complete Care Act assessments, safeguarding enquiries, Mental Capacity assessments, and contribute to Court of Protection work and Continuing Health Care (CHC) processes.

Ensure all care and support plans are outcome-focused, person-centred, and cost-effective.

Maintain a strengths-based approach throughout practice.

Requirements:

Minimum of 3-4 years' post-qualified statutory experience in adult social care.

Experience supporting adults with complex needs, ideally in a community setting.

Strong knowledge of the Care Act 2014, MCA 2005, and safeguarding procedures.
