Job Title: Qualified Social Worker - Adults
Location: The Shirehall, Shropshire Council
Rate: £28.40 per hour (Umbrella - capped rate in line with the MOU)

Contract Type: Locum
Working Hours: Full-time (with occasional weekend rotation)

Overview:
Shropshire Council is seeking a Qualified Social Worker to join the Adults Social Care Team. This is an exciting opportunity to support hospital discharges and ensure timely, person-centred interventions for adults with varying needs.

You will be working in a supportive, multidisciplinary environment with a focus on strength-based practice and compliance with the Care Act 2014 and Mental Capacity Act 2005.

Key Responsibilities:

Conduct assessments to determine individuals' short- and long-term needs following hospital discharge

Attend and participate in multidisciplinary team (MDT) meetings

Complete Care Act Assessments and Mental Capacity Assessments

Liaise with the ICB regarding Continuing Healthcare (CHC) funding, including interim health funding, CHC checklists, and DST meetings

Develop and implement support and care plans

Participate in community duty tasks on a rotational basis

Work one weekend every 6-8 weeks (Saturday and Sunday, 9am-1pm)
