Job Advert: Social Worker - Adults Hospital Pathway Team
Location: Sutton (St Helier Hospital & Civic Offices)
Pay Rate: Dependent on experience
Contract Type: Locum
Working Hours: Full-time, Monday to Friday

We are currently seeking two experienced Adult Social Workers to join the Hospital Pathway Team in Sutton. These roles play a critical part in facilitating timely and safe hospital discharges, completing statutory assessments, and managing complex cases post-discharge.

Role 1 - Based at St Helier Hospital
Full-time, fully onsite role (Monday to Friday)

Supporting discharges from hospital in collaboration with the Continuing Healthcare team

Completing assessments and reviews under the Care Act

Undertaking Mental Capacity Assessments (MCA) and Best Interest Meetings

Completing Decision Support Tools (DST) post-discharge

Managing complex and safeguarding cases

Role 2 - Based at Civic Offices (Hospital Pathway Team)
Hybrid working: 3 days in the office and 2 days from home

Carrying out Care Act assessments post-discharge under the Discharge to Assess (D2A) model

Leading on safeguarding enquiries and managing team safeguarding cases

Completing MCA and Best Interest decisions

DSTs post-discharge and supporting Court of Protection applications

Managing complex casework

Requirements:
Qualified Social Worker with registration with Social Work England

Strong experience in adult hospital discharge planning, safeguarding, and mental capacity work

Ability to work in a high-paced, multi-agency environment

Confidence in decision-making and managing complex and sensitive cases
