Location: County Hall, Trowbridge (Wiltshire)
Pay Rate: £35 per hour (Umbrella)
Hours: 37 hours per week
Working Model: Hybrid (50% from home)
Job Description:
We are looking for a passionate, motivated, and experienced Qualified Social Worker to join our Mental Health Team in Wiltshire.
This is a fantastic opportunity to be part of a small, well-established, and dedicated social care service supporting adults (18+) with severe and enduring mental health conditions. Our team focuses on promoting recovery, independence, and person-centred support, ensuring individuals have choice and control over their lives.
Key Responsibilities:
Assess, coordinate, and arrange care for service users with mental health conditions, including dementia.
Support individuals both in the community and hospital settings.
Work in an integrated way with local communities and partner organisations.
Promote independence and recovery, focusing on strengths-based approaches.
Requirements:
Registration with Social Work England (or appropriate professional body).
Experience working in a health or social care setting.
Excellent communication skills and a commitment to collaborative, integrated working.
Passion for mental health social work and person-centred practice.
