We are currently seeking an experienced and dedicated Qualified Social Worker (QSW) to join the Mental Health and Hospital Discharge Team in Wrexham. This is a fantastic locum opportunity offering a competitive hourly rate and the chance to make a significant impact within adult mental health services.
Key Responsibilities:
Undertake assessments and care planning under the Social Services and Well-being (Wales) Act (SSWBA).
Support hospital discharges in line with best practice and statutory guidance.
Complete Mental Health Act (MHA) and Mental Capacity Act (MCA) assessments where appropriate.
Work collaboratively with NHS staff, community teams, and service users to promote safe discharge and continuity of care.
Maintain accurate and timely records in line with statutory and service requirements.
Requirements:
Qualified Social Worker with current registration with Social Care Wales.
Proven experience in adult mental health and hospital discharge work.
Strong knowledge of:
Social Services and Well-being (Wales) Act (SSWBA)
Mental Health Act (MHA)
Mental Capacity Act (MCA)
Mental Health Act Manual / Management Procedures (MHAM)
Ability to work autonomously and as part of a multi-disciplinary team.
