Overview:
We are seeking a compassionate and motivated Band 5 Community Mental Health Nurse to join our Dementia Team, providing specialist support to individuals living with dementia and their families in the community.

Key Responsibilities:
Conduct comprehensive mental health and cognitive assessments.

Develop and implement person-centred care plans tailored to each individuals needs.

Provide support, education, and advice to patients, families, and carers.

Work collaboratively within a multi-disciplinary team to deliver holistic care.

Assist with risk management, safeguarding, and crisis interventions as needed.

Participate in case discussions, reviews, and care planning meetings.
