Job Title: Occupational Therapist - Hospital Team

Pay Rate: Up to £38 per hour
Working Pattern: Hybrid (Minimum 2-3 days per week in the office)

Job Overview
The Royal Borough of Windsor & Maidenhead is looking for an experienced Occupational Therapist to join their Hospital Team. The role involves delivering an outcome-focused assessment service to adults with physical disabilities or age-related needs, supporting safe and timely hospital discharges.

Key Responsibilities

  • Carry out outcome-focused assessments for adults with physical disabilities or age-related conditions.

  • Support hospital discharge planning in collaboration with health and social care professionals.

  • Work closely with multidisciplinary teams to ensure effective care planning.

  • Maintain accurate case records and documentation.

Requirements

  • Qualified Occupational Therapist with relevant professional registration.

  • Local Authority and hospital discharge experience required.

  • Knowledge of the Mosaic case management system is desirable.

  • Must be a driver with a valid driving licence and access to a vehicle for work purposes.

  • Ability to work hybrid, with 2-3 days per week in the office.
