Your key responsibilities will include:
- Conducting comprehensive assessments to determine the needs and risks of individuals being discharged from mental health facilities.
- Developing and implementing personalized discharge plans in collaboration with multi-disciplinary teams, clients, and their families.
- Providing ongoing support and guidance to clients post-discharge, including access to community resources and services.
- Maintaining accurate and up-to-date case records and documentation in accordance with relevant policies and procedures.
- Participating in team meetings, case reviews, and training sessions to enhance service delivery and professional development.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
- Registered with Social Work England.
- Proven experience in mental health settings, particularly in discharge planning and community reintegration.
- Strong interpersonal and communication skills, with the ability to engage effectively with clients, families, and professionals.
- Excellent organizational skills and the ability to manage a varied caseload.
