Eden Brown Synergy is currently recruiting for an experienced Qualified Social Worker (QSW) to join the Mental Health Hospital Discharge Team with Dudley Metropolitan Borough Council.
This is an excellent opportunity to work within a fast-paced, multi-disciplinary environment supporting timely and safe hospital discharges for individuals with mental health needs.
Role Details
Position: Qualified Social Worker - Mental Health Hospital Discharge
Location: Dudley
Rate: £31.97 per hour
Hours: 37 hours per week
Contract: 6-18 months
Start Date: ASAP
Key Aims & Objectives
Support service users and carers to achieve the shortest possible hospital stay
Deliver efficient, safe and person-centred discharge planning
Ensure individuals receive appropriate support to remain safely at home
Work collaboratively across health and social care to ensure seamless transitions from hospital into the community
Key Responsibilities
Complete Care Act assessments and provide appropriate recommendations
Undertake Mental Capacity Assessments
Attend ward reviews, pre-discharge meetings and Section 117 reviews
Liaise with housing providers, NHS teams and community support services
Work closely with Black Country Healthcare NHS Foundation Trust and acute hospital teams
Coordinate with discharge coordinators and partner agencies to ensure smooth care transitions
Present cases at funding panels and support Section 117 funding processes
Explore respite and step-down options where appropriate
Act as a key link between health and social care services
Ensure effective communication with locality Adult Social Care teams
Provide timely updates and escalate issues where required
Key Working Relationships
NHS organisations including Black Country Healthcare NHS Foundation Trust
Acute hospitals and community mental health teams
Adult Social Care locality teams
Housing providers
Voluntary, Community and Social Enterprise (VCSE) organisations
Requirements
Qualified Social Worker with SWE registration
Experience within Mental Health and Hospital Discharge
Strong knowledge of the Care Act, Mental Capacity Act, and Section 117
Ability to work within multi-agency settings
Experience attending funding panels and multi-disciplinary meetings
Strong assessment and care planning skills
