We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Discharge To Assess Team in Bristol. This pivotal role involves working collaboratively with health and social care professionals to ensure a seamless transition for individuals discharged from hospital settings. You will play a crucial role in assessing needs, planning care, and facilitating support for individuals to live independently in the community.

Key responsibilities include:

  • Conducting comprehensive assessments of individuals’ social care needs post-discharge.
  • Developing and implementing person-centered care plans in collaboration with service users and their families.
  • Working closely with healthcare teams to ensure safe and timely discharges from hospitals.
  • Providing information and guidance on available community resources and support services.
  • Monitoring and reviewing care plans to ensure they meet evolving needs.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., BA/MA in Social Work).
  • Current registration with Social Work England.
  • Strong knowledge of adult social care legislation and best practices.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
  • Demonstrated experience in discharge planning and assessment processes.
