Social Worker -Discharge To Assess Team- Bristol
A Social Worker is required for work within the Discharge To Assess Team in the vibrant city of Bristol. Excellent rates of pay up to £32ph. 3 month minimum contract but likely extended. 37 hours per week. Exciting opportunity in a great city with good transport links to neighbouring authorities. Hospital based social work role. Flexible working.
An excellent opportunity has arisen for an experienced Social Worker practitioner who has experience of working with Adults with a wide range of presenting needs. You will be based in a well- supported/managed team in a hospital setting based in Bristol.
The role requires you to work within relevant current legislation and collaboratively with adults and their families/carers to assess their needs and plan and deliver services in accordance with the social work team's service area. You will attend MDT's / flow hub meetings and complete safeguarding within the hospital setting. The successful candidate will have the ability to respond promptly, carry out proportionate assessment and recording, working closely with community colleagues and health partners as required.
You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK
