Join our dedicated Integrated Learning Disabilities (LD) Team in Hackney as a Qualified Social Worker (SW) to make a meaningful impact in the lives of adults with learning disabilities. In this role, you will be instrumental in assessing needs, developing care plans, and collaborating with multidisciplinary teams to ensure the best outcomes for our service users.

Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and support requirements.
  • Developing, implementing, and reviewing care plans in collaboration with service users, their families, and other professionals.
  • Providing support and guidance to service users and their families, empowering them to make informed decisions about their care.
  • Maintaining accurate and up-to-date case records in accordance with local policies and procedures.
  • Participating in team meetings and contributing to the development of best practices within the LD team.

To be successful in this role, you will need:

  • A recognized social work qualification (BA/MA in Social Work or equivalent).
  • Registration with Social Work England.
  • Experience working with adults with learning disabilities, preferably within a community or integrated team setting.
  • Strong communication and interpersonal skills to build effective relationships with service users, families, and colleagues.
  • Ability to work independently and as part of a team, demonstrating excellent organizational skills.
