We are currently seeking a skilled and experienced Advanced Social Worker to join the Hospital Discharge Neighbourhood Team in Leeds, supporting timely and effective discharges from a range of care settings.

Key Responsibilities:
Manage discharges from:

Out-of-area hospitals

Recovery hubs

Temporary care placements

Conduct care reviews and assessments in service users' own homes

Work closely with multi-disciplinary teams and health partners to coordinate discharge plans

Ensure person-centred, strengths-based practice is embedded throughout your work

Requirements:
Qualified Social Worker with current Social Work England registration

Demonstrable experience in hospital discharge and community-based adult social care

Knowledge of the Leeds CIS Apex recording system is essential

Ability to work independently in a fast-paced environment

Available for a 12-week assignment, with the potential for extension

Additional Details:
Role is ICB funded

Immediate start preferred

Competitive rate: £35 per hour (via umbrella)
