Overview:
Farmfield Hospital is seeking an experienced and compassionate Social Worker to join our multidisciplinary team supporting adult males in secure and acute mental health settings. The successful candidate will play a key role in promoting recovery, care planning, safeguarding, and discharge coordination in a complex but rewarding environment.

Key Responsibilities:

Manage a defined caseload of service users within secure and acute settings

Undertake mental health assessments and provide robust risk management plans

Facilitate and coordinate effective discharge and aftercare planning

Work collaboratively with clinical teams, external agencies, and families

Attend and contribute to ward rounds, CPA reviews, and multi-disciplinary meetings

Complete Mental Health Tribunal and Ministry of Justice reports

Support service users through the Care Programme Approach (CPA) process

Ensure safeguarding protocols are followed in line with legislation and best practice

Advocate for patients and uphold their rights in accordance with the Mental Health Act and relevant statutory frameworks

Requirements:

Qualified Social Worker with current Social Work England registration

Experience working within forensic, secure, or acute mental health services

Strong understanding of relevant legislation including the Mental Health Act, Care Act, and Human Rights Act

Excellent communication, documentation, and case management skills

Ability to work within a multi-disciplinary team and independently

Enhanced DBS clearance (or willingness to undergo one)
