We are currently seeking an experienced and motivated Qualified Social Worker (QSW) to support adult service users across North Lincolnshire. You will work as part of a dynamic team providing statutory support to adults with care and support needs, helping them maintain independence, safety, and quality of life in their own communities.

Key Responsibilities:
Undertake Care Act 2014 assessments, support planning, reviews, and risk management

Develop and coordinate person-centred care plans that promote independence

Complete Mental Capacity Assessments and contribute to safeguarding enquiries

Work with adults with physical disabilities, frailty, mental health needs, and those requiring hospital discharge planning

Collaborate with families, carers, and multi-disciplinary professionals

Ensure all practice is strengths-based, outcome-focused, and compliant with legislation

Keep accurate, up-to-date records and documentation

Requirements:
Social Work qualification and Social Work England registration

Experience within a Local Authority Adult Social Care setting

Knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks

Ability to work both independently and collaboratively within a team

Proficient IT and case recording skills (e.g., using Liquidlogic or similar systems)
