We are currently seeking a Qualified Social Worker to join the Ageing Well (North) Team in Nottinghamshire. This rewarding role focuses on supporting older adults to live independently and safely within the community.

Key Responsibilities:

Conduct strength-based assessments and care planning

Support hospital discharge and community interventions

Work collaboratively with health professionals and families

Manage safeguarding and complex casework

Requirements:

Social Work England registration

Minimum 2 years' post-qualifying experience in adult services

Strong knowledge of the Care Act, MCA, and safeguarding procedures
