We are seeking a dynamic and experienced Lead Practitioner to join our Adult Social Care team. This is a pivotal role in leading and supporting a team of professionals delivering statutory duties, safeguarding vulnerable adults, and embedding strengths-based, person-centred practice.

About the Role
As a Lead Practitioner, you will provide professional leadership, mentoring and guidance to social care staff, ensuring effective assessment and management of risk while driving forward personalised, outcome-focused support for adults in our communities.

You will be responsible for:

Leading and coaching staff to deliver high standards of care and support

Holding a small, complex caseload and providing oversight on high-risk cases

Driving service improvement, innovation, and transformation

Championing safeguarding and quality assurance standards

Collaborating with internal and external partners to ensure integrated working and efficient service delivery

Demonstrate strong leadership, decision-making, and communication skills

Bring extensive knowledge of social care legislation, safeguarding, and risk management

Support a high-performing and flexible workforce through mentoring and quality assurance

A recognised Social Work qualification, registration with Social Work England, and significant post-qualification experience within adult services are essential.
