Locum Qualified Social Worker - Transitions Team (Adults) - BANES
Location: Keynsham (Hybrid - 2 days office-based)
Rate: £39.99 per hour
Contract: Locum - Full-Time
Bath and North East Somerset Council (BANES) is seeking an experienced Qualified Social Worker to join the Adult Social Care - Transitions Team on a locum basis.
Key Responsibilities:
Undertake Care Act assessments, support planning, and Mental Capacity Assessments (MCAs) for young people transitioning to adult services
Work with a strengths-based approach to promote independence and choice
Collaborate with partner agencies to deliver effective, person-centred support
Conduct face-to-face visits, with a requirement of a minimum of 2 office-based days per week
Requirements:
Social Work England registration
Proven experience in Adult Social Care, preferably with transitions
Strong knowledge of the Care Act 2014 and Mental Capacity Act 2005
A car user is essential, as some clients live in rural areas
Ability to work both independently and as part of a team in a hybrid model
