About the Role:
As a Social Worker within the Initial Contact Team, you will be part of a friendly and collaborative environment that places high value on strengths-based and person-centred practice. This team provides a high-quality, responsive service at the first point of contact into Adult Social Care.

You'll be handling short-term interventions, assessing needs through strength-based conversations, signposting to community-based support, and arranging both short- and long-term care solutions. The team focuses on early resolution, aiming to promote independence and well-being through effective engagement.

Key Responsibilities:
Provide clear, concise advice and triage support at the initial point of contact

Complete timely and holistic assessments using strengths-based approaches

Manage a small caseload of complex cases

Arrange appropriate support to meet the identified outcomes of service users

Promote positive risk-taking and ensure all support plans are least restrictive and cost-effective

Work collaboratively with individuals, families, and multi-agency partners

About You:
Qualified Social Worker with current Social Work England registration

Experience in adult social care, preferably in a front-door/initial contact setting

Strong understanding of the Care Act, Mental Capacity Act, and safeguarding procedures

Ability to assess needs and coordinate care with a solution-focused approach

Confident, compassionate, and committed to promoting independence and well-being
