We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Learning Disabilities Team in Hackney. Your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs of adults with learning disabilities.
- Developing and implementing person-centered care plans in collaboration with service users and their families.
- Providing ongoing support and guidance to individuals and their families to enhance their quality of life.
- Working closely with multidisciplinary teams, including healthcare professionals, to ensure holistic support.
- Advocating for the rights and needs of service users within various settings.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Current registration with Social Work England.
- Experience working with adults with learning disabilities, including knowledge of relevant legislation and best practices.
- Strong communication and interpersonal skills, with the ability to build rapport and trust.
- Excellent organizational skills and the ability to manage a varied caseload effectively.
