- Conducting assessments and reviews of care plans, ensuring they meet the needs of service users.
- Collaborating with external agencies and stakeholders to enhance service delivery and promote integrated care.
- Participating in training and development initiatives to enhance team performance and service quality.
The ideal candidate will possess essential qualifications and skills, including:
- A relevant social work qualification (e.g., Degree in Social Work, DipSW, CQSW).
- Registration with Social Work England.
- Significant experience in adult social care, with a strong understanding of relevant legislation and best practices.
- Excellent communication, leadership, and interpersonal skills.
- Ability to work collaboratively within a multi-disciplinary team.
