We are currently seeking a dedicated and compassionate Qualified Social Worker to join our Warwickshire Adults Learning Disability Team. As a member of this dynamic team, you will play a crucial role in supporting adults with learning disabilities to lead fulfilling and independent lives.

Key responsibilities and duties:

  • Assessing the needs of individuals with learning disabilities and developing care plans tailored to their specific requirements.
  • Providing practical and emotional support to service users and their families to enhance their well-being and quality of life.
  • Collaborating with multi-disciplinary teams, including healthcare professionals and social care providers, to ensure holistic care and support.
  • Advocating for the rights and interests of individuals with learning disabilities to promote their inclusion in the community.
  • Conducting regular reviews and assessments to monitor progress and make necessary adjustments to care plans.

Essential qualifications and skills:

  • Qualified Social Worker with a valid registration with Social Work England.
  • Demonstrated experience working with adults with learning disabilities in a social care setting.
  • Excellent communication and interpersonal skills to engage effectively with service users, families, and colleagues.
  • Strong assessment and case management skills to provide person-centered support.
  • Ability to work autonomously and as part of a team in a fast-paced environment.
